Living Choice Program Case Management Training
What is it about?
This 3-hour training provides information about the Living Choice Program (LCP) and the steps and processes for smoothly transitioning a Member from an institutional setting to a community residence of their choice. Training topics include:
- Introduction to Living Choice Program
- Eligibility requirements – LCP vs ADvantage key differences
- Describe the services available to Members who transition
- From referral to community living, the LCP transition workflow
- Tools for successful service plan development, implementation, transition, stability and monitoring
- Transitional Funds reimbursement
- Alternative Funds reimbursement process
- Assisted Living (room and board and vendor pay)
- Roles and responsibilities of the LCP, Member, Case Manager, and informal supports
Who should attend?
Registration for this training is open to all active ADvantage Case Managers and Case Manager Supervisors.
Prerequisite: minimum of six months experience as an ADvantage Case Manager
Why should I attend?
Effective July 1, 2025, Providers will be required to employee at least one LCP certified case manager and one LCP certified Case Manager Supervisor. Successfully completing this training earns the required Living Choice Program case management certification.
