Medically Fragile Waiver (MFW) Case Management Certification Training

What is it about?

This 3-hour training course provides information about the steps and processes for smoothly transitioning an applicant or Member to the Medically Fragile Waiver. Training topics include:
  • Introduction to Medically Fragile Waiver.
  • Eligibility and Program requirements
  • MFW services available
  • Referral and enrollment process
  • Financial qualifications and responsibilities
  • Roles and responsibilities of the MFW, Member, Case Manager, and the informal supports
  • MFW service specific documentation, UCAT, Harmony notes, service plan development, authorizations and addendums
  • Monitoring, back-up plans and critical incidents
  • Frequently asked questions and time savers
Registration is open to all active ADvantage Case Managers and Case Manager Supervisors.
Prerequisite: minimum of six months experience as an ADvantage Case Manager.

Why should I attend?

Effective July 1, 2025, Providers will be required to employee at least one MFW certified Case Manager and one MFW certified Case Manager Supervisor. Successfully completing this training earns the required Medically Fragile Waiver case management certification.
